2026 Ag & Art FARM Participant Form

Please fill out this form to the best of your ability. Participants will be contacted by Christian Cooke or Tracy Cornett upon application approval. A $100 fee is required to participate in the tour, which includes paid promotional materials for your business. Participants who attend the tour will be refunded $50 after the tour.
